most people were labourers (farmers, blacksmiths etc) so they would always get up with the sun. After so much time of doing this, their bodies would get used to it and so it would come automatically, even on overcast days.

long time ago ; when humans were living in the caves there was different choices like "el hombre" with a budgeon or the bear or the wolves searching for their food or, let me think whatelse, ah right "la chica" searching for el cabalhero;

People would get up (and go to bed) with the sun. No staying up late playing on the computer!

In towns, some churches would have a clock that would ring out the time. Church bells would also call people to come to the church for prayer.

A lot of people who work everyday at the same time will have the "Internal" alarm clock. As a construction worker I’m up every day at 4 and I will wake up a couple of minutes before the alarm every day, even the days i don’t have to get up. Also, if you’re bed is fron of the Window, the sun will wake you up. That rooster, he crows all night, all the time, it’s just that when you are about ready to wake his next crow will be the one to wake you. That’s an old wives tale
